TECHNOLOGICAL ASPECTS OF CREATING SOFT DOSAGE FORMS FOR THE TREATMENT OF PURULENT WOUNDS

Abstract

Introduction. Treatment of wounds and wound infection is an important issue for the military medicine system and the national health care system of Ukraine. At the present stage the main approaches in the purulent wounds treatment are the pathogenetic orientation in accordance with the phase of the wound process. It includes: debridement, additional treatment of the postoperative wound, early closure of the wound defect, wound drainage, antibacterial therapy, elimination of the factors that slow down healing. The leading role in topical treatment belongs to soft dosage forms – ointments, creams, gels, which provide high therapeutic efficacy antiseptic, antibiotics, anti-inflammatory and reparative drugs in the affected area with minimal side effects.

Purpose of the study - analysis of modern technological aspects of creating soft dosage forms for the treatment of purulent wounds.

Materials and methods: national and international scientific literature used in the work; methods of systematic analysis and biblical semantic research applied in the work.

Results.The approaches of the State Pharmacopoeia of Ukraine, the European Pharmacopoeia, the US Pharmacopoeia and the integrated classifications proposed by a number of scientists to the classification of soft dosage forms are summarized.

The main pharmaceutical factors influencing the efficacy and quality of soft dosage forms are determined: physical properties of active pharmaceutical ingredients and excipients, nature and amount of base-carrier and excipients that are part of the drug, type of dosage form and technological operations carried out during its manufacture.

Bases-carriers are important components of soft dosage forms, as they account for 90% or more of the total weight and affect the activity of the pharmaceutical ingredients and the rheological properties of the dosage form. It is determined that in accordance with the requirements of the State Pharmacopoeia of Ukraine, the classification of the bases of soft drugs is carried out on the basis of affinity for water and the type of dispersed system. The US Pharmacopoeia uses additional differentiation by the ability to absorb water, which leads to the allocation of four classes of bases for ointments: hydrocarbon, absorbent, water-washable and water-soluble.

The analysis of research results showed that clinically justified for the local treatment of the wound process is the use of ointments based on hydrophilic and emulsion bases, which are characterized by dehydrating action and the ability to effectively conduct active pharmaceutical ingredients to wound canals and cavities.

Conclusions.  Soft dosage forms have a leading role in the local treatment of purulent wounds, as they ensure the achievement of the optimal concentration of active pharmaceutical ingredients in the affected area, providing high therapeutic efficacy and prolonged action with minimal side effects of the components. Pharmaceutical factors are crucial for the development of soft dosage forms for the treatment of purulent wounds: the physical properties of drugs, the nature and amount of base-carrier and excipients that are part of the drug, the type of dosage form and technological operations performed in its manufacture. At the present stage, it is advisable to develop complex drugs for certain phases of the wound process, which can simultaneously affect several factors of pathogenesis. It is determined that for the treatment of purulent wounds in the first phase of the wound process it is advisable to use soft dosage forms with strong and prolonged osmotic activity, in the second phase - with moderate osmotic activity, in the third phase of the wound process dosage forms should prevent drying of the wound surface, protect and stimulate growth of granulations.
